We inform  you that named Uwitonze Claudette, son/daughter of Gafaranga and Dushimirimana , domiciled at Nganzo village, Rwebeya cell, at Cyuve Sector,Musanze District,Northern Province applied for the authorization of the change of name UWITONZE CLAUDETTE to be Called UWITONZE  THEODETTE in the National Population Registry.The reason for the Change of name is that is a name  given to me at the birth but was not registired in the Population Registry.
